Community Health Representatives
Our Community Health Representatives (CHRs) serve as a vital link between healthcare providers and the community, helping improve access to care. They assist with transportation to medical visits, provide outreach, education, and social support, and help patients navigate health services. CHRs are dedicated to improving well-being, advocating for patients, and reducing barriers to care through culturally responsive, compassionate support. *CHRs are able to assist members of our seven consortium reservations.
Patient Advocates
Patient Advocates are dedicated to guiding patients through their healthcare journey by offering personalized support and advocacy. They assist with scheduling appointments, understanding medical information, navigating health plans, and addressing concerns. Patient Advocates remove barriers to care, ensuring patients feel heard, informed, and empowered to make decisions that support their overall well-being. *Patient Advocates are able to assist all SIHC patients.
